Root Cause Analysis

Root Cause Analysis (RCA) is a structured investigation that gets past the obvious immediate cause of a failure to the underlying systemic reason it was allowed to happen — so the fix prevents recurrence rather than treating a symptom.

The OpsPilot RCA module runs the investigation as a guided conversation across nine stages: problem statement, business impact, evidence, an interactive Ishikawa fishbone, 5-Why drill-down, root-cause confirmation and validation, an action plan with owners and dates, and finally the report. Every finding is tagged for confidence — CONFIRMED, ASSUMED or INFERRED — so fact and opinion stay separate.

It applies recognised methods (Ishikawa fishbone, 5-Why, Pareto ranking) with the ISO 14224 equipment taxonomy, and produces an audit-ready Word report. As with every OpsPilot output, the report is a draft for a competent person to verify before it is relied upon.
